The 12-point quality gate

01 — Objective

Does the output actually answer the task you gave it?

02 — Facts

Are factual claims supported by the information available, or clearly marked when uncertain?

03 — Assumptions

Has the AI silently turned an assumption into a fact?

04 — Missing Context

Is anything important missing that could materially change the answer?

05 — Source Fit

If sources or files were used, does the output reflect what they actually say rather than what the AI expected them to say?

06 — Freshness

Does any claim depend on current information that should be checked again?

07 — Capability

Is the AI claiming it accessed, changed, sent, saved, remembered or verified something it could not actually access or do?

08 — Consequence

Is the confidence and reasoning depth appropriate for the real-world consequence of being wrong?

09 — Completeness

Are the required deliverables, constraints, fields, steps and edge cases present?

10 — Simplicity

Has unnecessary complexity, architecture, jargon or process been added?

11 — Actionability

Can a real person tell what to do next?

12 — Final Human Check

Would you be comfortable attaching your own name to this output?


Four useful verdicts

PASS

Safe enough for the intended low or normal-risk use after your review.

REVISE

Useful, but one or more material issues need fixing.

VERIFY

External or source verification is required before use.

REJECT

The output is fundamentally wrong, misleading, unsafe or not fit for purpose.
